Emergency Contact Information

In the event a child is involved in an accident, becomes sick, or any other emergency while at SEEDS, it is imperative we have on file the name(s) of the person(s) you would want to be contacted. We will reach out to each person listed until we reach someone. We therefore require each family to complete the information requested below.

If you have more than one child attending camp, you need only fill out this form once if the emergency contacts are the same for each child.
