Summer Camp Waivers
Below you will find a liability waiver, a transportation waiver, and a photo/video release waiver for your camper. Read each part carefully and sign at the bottom of the page. Please complete this form for each child attending camp.
Part One: Liability Waiver
I indicate my agreement, and that of my child, to waive, release and hold harmless SEEDS and their volunteers, employees, and board members from any claims or legal action resulting from the activities related to SEEDS.
I hereby release and agree to hold SEEDS harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of SEEDS, or that may otherwise arise in any way in connection with any services received from SEEDS.
I understand that this release discharges SEEDS from any liability or claim that I, my heirs, or any personal representatives may have against SEEDS with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from SEEDS.
In the event that my child becomes ill or sustains an injury while at SEEDS, I give my permission to administer first aid to my child. If I cannot be contacted immediately in the event of an emergency, I authorize SEEDS to consent to emergency hospital care for my child. Should any illness or accident occur to my child, I will not hold liable the representatives of SEEDS. I assume full responsibility for all related medical costs.
This liability waiver and release extends to SEEDS together with all owners, partners, and employees.
Part Two: Transportation Waiver
I indicate my agreement, and that of my child, to waive, release, and hold harmless SEEDS and their volunteers, employees, and board members from any claims or legal action resulting from transportation provided to/from SEEDS related activities by SEEDS employees.
Part Three: Photo and Video Release
I grant SEEDS, its representatives and employees the right to take photographs of my child during program activities and authorize SEEDS to use and publish photos of my child in print and/or electronically for documentation or publicity purposes.
