Pick-Up Release Form

In the event a child is not picked up by the enrolling parent, and is to be picked up by another adult, it is imperative we have on file the name(s) of the person(s) you would want to be the child to be released to. We will not release the child to any person not on the form, regardless of relation.

If you have more than one child attending camp, you need only fill out this form once if the pick-up people are the same for each child.
